I think that confidentiality is the gold standard. The duty to report/ protect arise only if it is for the benefit of the victim or society. As to the duty to protect (report) and confidentiality, in my state of New Jersey, if the provider (health professional, teacher, Stephen Ministry care-giving volunteer in a church, etc) has reason to believe that some harm is underway, then the professional has a duty to report it to the authorities and it appears that all confidentiality goes out the window.
